Available for home visits to assess needs and coordinate services

Ensure Nutrition Shakes provided at a discount or at no charge with a physician's prescription [application]
Farmers' Market Healthy Seniors Program
Farmers' Market coupons to be used for fresh local produce for income eligible seniors [application]
FILE OF LIFE
Magnetic pouch for the refrigerator and wallet insert to record medications and medical conditions for first responders
Food Commodity Program
Partnering with CASC to provide monthly food boxes for income eligible seniors through a Federal Program
Heat Relief
Air Conditioning units provided through Community Action for income eligible seniors with a physician's referral ; fans provided to income eligible seniors

Assistance with meal preparation, light housekeeping and laundry to maintain a clean, safe and healthy environment

Funding provided to the Salvation Army to assist low income seniors at risk of becoming homeless

Providing ramps and a variety of home repairs through a cooperation with St John's Lutheran Church of to improve independence and safety

Providing resource information about community-based and other services for older adults
Library @ Your Door and Visitation Program

Life-Bios - Life Story Journal
Journal to document the life of a senior as well as an instructional class or the option to pair with companion visitation
Medicare Assistance
Medicare counseling by Senior Services' OSHIIP-certified (Ohio Department of Insurance) staff

Medication set-up by a registered nurse for qualified seniors
Mental Health Specialist / Senior Well-Being Counselor
Senior Mental Health Coordinator contracted through Consolidated Care, Inc

Assistance in the performance of necessary activities, such as personal hygiene and grooming
Project Lifesaver
Funding provided to the Union County Sheriff's Office to support tracking system for those at risk to wander such as Alzheimer's and Dementia clients
Project Safe Heat
Free carbon monoxide detector, furnace cleaning and inspection for seniors through the Community Action office
Senior Shopping Cart
Grocery shopping service available for those medically unable to shop for themselves with a physician's or social worker's referral [application]
Transportation to the grocery store available for those who don't drive through UCATS
Senior Watch Telephone Call Program
Daily automated call program for safety operated by the Union County Sheriff's Office
Smoke Detector Program
Smoke detectors provided to local Fire Departments for Union County Seniors

Medical and Community Meals transportation available; wheelchair transportation available through UCATS
Union County Family YMCA Senior Sunrise
Funding provided to support Senior Sunrise, free admission to the YMCA Friday mornings
Volunteer Friend Program
Visitation from a non-professional through the Volunteer Guardian Program for moral support to assist a senior to remain independent
Volunteer Guardian Program
Court appointed guardian program for those the law regards as incapable of managing their own affairs
Welcome Baskets for Seniors
Welcome baskets delivered to seniors new to the county by Union County Senior Services staff that contain senior-related community information and give aways
